For new worship leader licenses.  Registration is mandatory and the registration form (click here) MUST be completed and returned to the diocesan office to participate.

Instructions

Cost: $15.00 to cover cost of materials and lunch. Fill out the attached form and fax, email or snail mail it to the diocesan office. Address is listed on the form. To pay by credit card click the "Register Now" button at the bottom of the page.
